TraCS Form Types

Most forms have a designation that will show in the Form Type column in the Forms Manager once the form is filled out.

Attachments - Whatever is typed into the Attachment Name field on the form will show under Form Type, for example "Witness Statement - Jones."

Baker/Marchman Act - Baker or Marchman

Boating - Citation or Warning

Crash - D, L, S, or U

Driver Exchange, Long Form, Short Form, Update

DUI Citation - C

Criminal

FIBRS Offense Report - O or S

Original or Supplement

Parking/Ordinance/Civil Citation - Citation or Warning

Traffic Warning - B, C, N, M, or P

Bicycle, Criminal, Non Moving, Moving, or Pedestrian

Uniform Arrest Affidavit (UAA)- Arrest Affidavit, Juvenile Referral, Notice to Appear, PC/Sworn Complaint, or Request Capias

UTC - B, C, N, M, or P

Bicycle, Criminal, Non Moving, Moving, or Pedestrian

Daily Observation Report (DOR), DUI Refusal, FIR, Implied Consent, Tow, and Trespass forms do not have a Form Type designation.
